Heartbroken over the fact that my sweet Father in Law spent the last days of his life at Apple Rehab in Guilford, CT. As if having to place a loved one in care during COVID was not difficult enough, the care he received and the communication with the family were poor at best. He was often unkempt and wearing strangers clothing during outdoor visits, if he was fully clothed at all. His personal items were lost including his personal Bible, which was eventually found. What was not recovered was his wedding ring which Yale New Haven Hospital documented that he did have with him when he was transferred to Apple. After their "internal" investigation they concluded he did not have it on him. When the family was finally able to go in to see him as he was dying they were shocked by the disheveled, rundown and dirty condition of the facility itself. Guilt over the fact that this is where he spent his last days will be with us for a very long time. However, the most tragic part of this situation was the lack of direction, advocacy or follow up to place him on Hospice AS REQUESTED by the family to allow personal in room visits as he was rapidly declining. A plan was in place to do so, but after staff advised my 90 year old mother in law that he was ''improving'' the plan was reversed. He died alone in a depressing environment. I have spoken to staff and administration and they stand by his care with no acknowledgement or apology. We will have to come to peace with the decisions we made, but I hope this review prevents the same for another family.

All the apple rehabs are absolutely disgusting and run down especially Guilford. You would think all the money they get from the residents they would make it more comfy and up to date being in GUILFORD instead of having old furniture and cabinets that are falling apart . The administration has no compassion or professionalism except for Cheryl . And for the people that think of sending a loved one there , I wouldn’t send my loved one to Apple because they are always understaffed which is why it’s difficult to run to a resident that is calling because the aides are tied up with another resident and the aides end up burnt out. The nurses don’t always document when a resident falls down or gets injured and when you have a trusting worker that tries to report it the administrator or nursing director is to busy to hear it. It’s very unsafe and not trustworthy enough. I wouldn’t send my worst enemy there .
Read moreGreat looking facility with amazing staff.......My dad was sent there for Covid and they took care of him and helped him to recover. The staff was great and very caring. I was able to visit him through window visits. Would defiantly recommend this Nursing home.
My loved one is dead because of this facility. They completely missed a gangrenous, malodorous ulcer that went all the way to the bone and it led to septic shock and death. My loved one is dead because of this facility. They completely missed a gangrenous, malodorous ulcer that went all the way to the bone and it led to septic shock and death. I can’t even get an attorney to agree to take the wrongful death case because the corporation that owns the facility, Consulate Health Care, was recently charged with MASSIVE Medicare fraud and now the company is almost certainly about to announce bankruptcy. Turns out, there are many others in the same boat. To anybody reading this, I am begging you....DO NOT put your loved one in the care of Hart’s Harbor. If you have a loved one already here...TAKE THEM OUT and transfer them elsewhere before you become a victim too. It is literally a matter of life and death.

They seem to give "canned" responses to your questions, but nothing really changes. Simple things like providing a longer telephone cord so she could reach the phone. I''m out of state and wanted to call her daily, but was unable to connect with her. My mom was not provided the care she truly needed. Stayed days in the same clothes. Unfortunately, we had to move her to rehab while awaiting a more SNF that would care for a dementia patient (although they tell you they have dementia patients.) My mom was not checked as frequently as she should have been; they took her off a medicine that she was put on to help her with delusions, the family was not told this. We only knew it after we started seeing changes in her, once she left the hospital. We finally asked and was told she was taken off of it. Why??? Then came the awful phone call to tell us she had fallen. My mother was on the floor for hours before anyone had checked her room to notice. Then she had a UTI, it was only then that WE insisted she be sent to the hospital. My mom passed away shortly after returning to the hospital. Apple did not provide the standard of care needed to treat my mom while she was there. I would not recommend this place.

This is a big “No.” When I arrived with my father to drop him off, no one came to the door. Once I realized ringing the door bell wouldn’t work, I had to call twice to get someone to answer the door. I didn’t know it at the time, but they failed to do an inventory of his personal items. Often during my father’s stay I would call and no one would answer the phone. After my father’s stay, I went to pick up his belongings, and all the money from his wallet was missing—approximately $50.00. I reported the theft to the Guilford police, but basically the end result was Apple was going to do an internal investigation. I still have not heard anything.
