We placed my mom at Brookdale in April of 2022. All went well until the Executive Director and RN both quit in July of that year. From that point forward there were numerous problems: frequent "interim" management filling in; and the RN wasn't filled until January of 2023. My mom's room was often very dirty, her dietary guidelines (diabetic) and certain meds were not being followed consistently, extremely poor communication due to the high turnover in management, missing laundry and linens for which reimbursement was never made to us despite being promised. In April 2023 We ultimately moved my mom to another memory care facility 15 minutes away, and she is now doing great and happier with much better care and cleanliness. I'd give Brookdale a one star rating except for the Activity Director, who was very good at her job. Thus the 2-star rating.

Brookdale Sun Prairie only offered memory care. I thought that they offered good information, provided an opportunity to follow up by sending me an email afterward, and a willingness to answer current and future questions. It was pretty small and intimate, but the staff seemed to know each resident. The person giving us the tour was pleasant and offered lots of good information. The negative side of it for me was it seemed like a dated facility, and the population that they were currently serving all seemed more advanced in their dementia journey. It didn't have any onsite rehab services. They all would be brought in if needed. With the folks that we observed, I didn't think that my father would have many opportunities to strike up communication with anyone and maybe some of the activities would not be as appropriate for him at his level as would be for someone more advanced. The rooms were fairly small and none of them had showers within the individual room. There was one shower room that would be for all residents to use. The layout was OK with the community spaces in the middle and then the individual rooms kind of off on the wings. Because it was smaller, there wasn't a whole lot of room, as if somebody wanted to be walking around. There was only one kind of living area/common area where people could comingle. The rooms were all studios.

The staff at Brookdale Sun Prairie was very thorough. Anthony was just on top of his game. He knows a lot about everything that goes on there. He's a really good representative. I sat down and ate with the residents. I had a mask on, but you could smell the food and it smelled amazing. I was head over heels with that part of the tour. Everything was fine. Everything was in order. It just was a good fit. Anthony was there to answer any questions. He took the time, and he wasn't rushed in any way. He just made very good use of his time and my time.

There are always people smiling and encouraging residents when I visit. The food is yummy and staff to resident ratio is more than adequate. My brother has a very large, friendly dog.
