Excellent care first class service I thank their nurses and Doctors for being patient and caring and the I would recommend sending my family to Crescent Cities Center
It started at a 5 and the longer I stayed there the lower the score dropped 😒. Wouldn''t put my worst enemy in there. Some of the workers take pride in their work but they are few and far between.

One of the worst places to be in for rehabilitation or nursing home. It takes two or sometimes three hours to be changed when you go to the bathroom. A couple of the staff said they are only required to change you twice in an eight hour shift and get mad and will ignore you if you go to the bathroo...
One of the worst places to be in for rehabilitation or nursing home. It takes two or sometimes three hours to be changed when you go to the bathroom. A couple of the staff said they are only required to change you twice in an eight hour shift and get mad and will ignore you if you go to the bathroom more than twice. I''ve been here since April and only seen and talked to the doctor once. The food is horrible they feed you the same thing every other day. They serve you cold food and some of the staff get mad if you ask them to heat up your food. The speech therapist lies and says she is going to change your food type so she can continue to work with you and get paid. The man over her also lies & says he will send a replacement in a week and it''s six weeks later and nothing happened. They only allow two showers per week and that doesn''t happen because the staff always complains that they are too busy or short-staffed. I haven''t had a shower in two weeks. My hair has not been washed in two months. I am unable to walk otherwise I would do it myself. Some of the nurses are rude and can take up to two hours to give you pain medication. Therapy is mediocre they rush you to get there and when you do they put you on a bike for ten to twenty minutes. You do arm lifts and leg exercises and stand up and that''s it. Then you are rushed to leave because staff wants to leave. The shift nurse Supervisor never answers the telephone day or night instead they push the ignore button or turn the ringer off. When you complain to management nothing happens or changes. I had a roommate that could barely speak and they left her laying in feces for hours and she ended up with an infection and ended up being transferred to the hospital. Do no put people you care about in this place unlessy theyo can speak up or you will regret it. There is a couple employees that do a great job and care about the patients.
