Park Ave care is a very nice place. Staff gave exceptional care in my case. All my questions were answered in a timely manner. I would recommend this place to everyone
Very pleased. The hospital sent my grandmother here for short term rehab. I must say we were very apprehensive about bringing bringing her to a nursing home but the rehab was outstanding . They got her walking in 2 weeks. The nurses were all very attentive. She didn’t particularly like the food so...
Very pleased. The hospital sent my grandmother here for short term rehab. I must say we were very apprehensive about bringing bringing her to a nursing home but the rehab was outstanding . They got her walking in 2 weeks. The nurses were all very attentive. She didn’t particularly like the food so much so we brought her food from home. Overall, we would recommend this place and bring her back if she ever needed medical help again.

My experience with this facility could have been better. The care was very poor. They need to be more accountable from leadership to frontline staff. My debilitated husband went in for rehab. Instead of rehab or restorative care, after three weeks, he looked like a skeleton due to a 40 lbs weight loss. When the institution was asked how this could have happened under their care, the family was told the scale was not working. Really? How could you have a broken scale in circulation? Clearly, they were not doing weekly weight checks. Hence, there is a lack of intervention or the formulation of a care plan to address any additional weight loss. Additionally, he fell several times while in their care. Understandably, staffing is a challenge. However, I need help understanding where the staff was as he languished on the floor, calling for help. There is a significant delay in call bell response time. Finally, we got tired of their poor care and decided to sign him out AMA. It was at that time that the administrator chose to intervene. Not only was it a waste of time, I found her attempt at service recovery laughable. She was condescending and dismissive. I mentioned in the meeting that he came in for rehab, and instead of rehabilitation, he kept falling in their care. Her response "he fell in his room." Seriously!! That''s the best answer she had to offer. In short, I do not recommend this facility.
